As a provider of creative care management and personal care services, we aim to bridge the gap between an aging population of seniors and the loved ones who take responsibility for their care.
Through a comprehensive assessment, our proprietary approach aligns the resources and support needed to reduce stress, rising costs, avoidable hospitalizations while improving the quality of life for both the senior and the family caregiver.
Our agency and direct staff are not only educated and trained in geriatrics care, most importantly we share our passion for delivering compassionate elderly care services. Our goal is to serve as your family's advocate in all matters relating to, but not limited to care coordination, in home support services, Medicare/ Medicaid issues, legal matters, financial services, and placement.

Our Services
Structured Family Caregiving
Structured Family Caregiving in Indiana offers financial assistance and support to caregivers. This assistance is available to both family members and non-family members. It also provides unskilled respite to caregivers during times of need. This program believes in keeping family members in their own homes or with caregivers instead of moving them to nursing homes. It promotes the idea of providing care for family members in familiar settings rather than institutional settings. This program advocates for the importance of maintaining familial connections and support systems in caregiving. It encourages families to consider alternatives to nursing homes for their loved ones. Caregivers in this program receive a fair daily stipend. The stipend is tax-free. This is to recognize and appreciate their hard work and dedication. Indiana Family Caregivers are paid the HIGHEST RATES in the state for our SFC clients. *Payment varies depending on the level of care approved by case manager.

Medicaid-Only Transportation
Non-Waiver/Medicaid-Only members who need a ride to a doctor’s appointment, pharmacy or other medical office can request transportation through WellTrans or LCP. Ask for Minty’s Haven Inc as your preferred transportation provider.
Attendant Care
The Attendant Care program, also known as Home Maker in Indiana helps pay caregivers including family members or close friends. Attendant Care is meant for those with disabilities and seniors needing daily care. This program ensures that eligible caregivers receive compensation for their invaluable services on an hourly basis. People in the Attendant Care program can help without living with the person they care for, giving more options for caregiving. It is important to remember that income earned from caregiving services is taxable. Caregivers should keep this in mind as they fulfill their caregiving duties. *Payment varies depending on the hours of care approved by case manager.
Waiver Transportation
If you don’t drive or you have a disability that makes accessing public transportation difficult, getting where you need to go can be a challenge. Residents of Indiana who have an approved Medicaid waiver for transportation services are eligible for transportation services within the home county. Ask for Alms Home Care LLC as your preferred transportation provider.
Caregiver Qualifications:
All primary caregivers must have a clean background check and go through agency and state approved training.
Who Is Eligible?
Live in Indiana
You must live in the State of Indiana and need long term care assistance with 2 or more ADLs (Activities of Daily Living). Hoosier Caregivers must be 18 years or older.
Have Medicaid and Get On a Waiver
Qualify for Medicaid or already have Medicaid and meet the financial and asset requirements for the Medicaid Aged & Disabled Waiver. (Our agency will help you through process)
Live Near or With Your Caregiver
For the Structured Family Caregiving Indiana Program, you must live in the same home as the Caregiver. For the Attendant Care Program, your caregiver just needs to live nearby.
